What to Bring for Pediatric Visits
Patient Registration Card
Health insurance card or My Number card
(*required at the first visit of each month)
Maternal and Child Health Handbook
Medication Notebook
(*not required if you do not have one)
How Your Visit Will Go

Check-in
Please present your child’s health insurance card or My Number card at reception.
If you have an appointment, provide your child’s name and scheduled time to the staff.

Waiting Area
Wait in the designated area until your child’s name is called. If this is your child’s first visit, you will be asked to complete a medical questionnaire.

Consultation & Examination
When called, please accompany your child into the consultation room and explain their symptoms in detail.
Dr. Nao Oikawa, the Deputy Director, is in charge of pediatric care.
Depending on the symptoms, further tests may be conducted.

Prescription & Payment
You will receive your child’s prescription at reception and proceed with payment.
In addition to cash, PayPay is accepted.
